KINSHIP CAREGIVER LEGAL ASSISTANCE WORKSHOP

Are you a grandparent or other relative? Do you need assistance enrolling a 
child in school or making medical decisions for a child that lives with you? Has your grandchild, niece or nephew been living with you for some time, and it is in the child’s best interest to remain living with you, but you don’t have legal authority to permanently raise the child? Maybe it’s time to apply for legal custody. We can help!

Come and receive hands-on assistance with filling out and completing legal documents for a Grandparent Power of Attorney, Caretaker Authorization Affidavit and/or Applications to Determine Legal Custody. Receive one on one legal advice from on-site attorneys.

REQUIREMENTS:
• Bring with you a birth certificate copy for each child
• Social security card copies for each child or a refusal letter 
• Also bring along Proof of Paternity for each child (e.g. marriage license, CSEA document, copy of court order, or Paternity Acknowledgement registry number)
• For a Grandparent Power of Attorney, both parents must be present. If only one parent can be present with the grandparent, reasonable efforts must be made to locate and provide notice to the other parent, and you have been unable to locate that parent; unless the other parent is prohibited from receiving notice, or the parental rights of the other parent have been terminated by order of a Juvenile Court
